Basically just them as magical girls. Most characters are genderflipped.
Quirks and heroes don't exist, but magic and folklore does. There are familiars, but that's a human term that's kind of belittling since that's like saying their entire purpose is to bond to humans and the friendlier term is spirit. They can be anything form yellow rabbits to filing cabinets with eyes that grant blessings or curses to people or bond with them and become their patrons. Sometimes, if a spirit bonds to a certain family multiple times, people born in that family learn how to do magic on their own that's similar to the patron's blessing or curse and sometimes kids are born spirits to human parents.
Magical girls became a thing when good familiars wanted to stop bad familiars and humans and it's mostly girls because they're a bit misogynistic and thought that girls are more pure and trustworthy. Eventually being a magical girl is a normal job now with monetary compensation by the Magical Girl Commission which is run by familiars and humans working together. Nezu is high ranking up there and he runs UA to help teach kids how to be good magical girls as well as offer decent education.
Sometimes unbonded spirits join up to learn to be heroes even without human partners although sometimes after graduating they just stick around the school to wait for a good partner to show up. Sometimes boys can join although usually they're not chosen as magical girls, they just did something else, like how Tokoyami bonded with a demon that gave him a bird head and said demon living in his body as a corporeal shadow.
Magical girl schools have morality tests in the entrance exam to make sure that only good people get in and sometimes people without patrons can enroll in the school on a trial basis and if they prove themselves by their equivalent of the Sports Festival, then a familiar can choose to mentor them and let them be magical girls.
Also, wow, decent governing system, I didn't know those were possible.
Todoroki has a better family system because her dad wanted to be a magical girl but couldn't because he's a boy and the morality test showed some mildly concerning behaviors that led him to be cut. Enji married Rei out of love and had normal kids and Touya wanted to be a magical girl too but also failed for similar reasons. Todoroki says that her dad's more or less decent but just feels sometimes that he pushes his kids to attain the thing he wasn't able to and fulfil his dream for him, even if it's not maliciously.
And now we have everyone's magical girl names because I hyperfixated on this too much before immediately dumping it.

Setsuna separates into lizards/dragons instead of body parts but they still somewhat look like body parts Hydra
Komori knows mushroom magic and has no patron due to one of her ancestors bonding with a mushroom spirit

Katsuki's a girl and she's a good friend. Deku's just an affectionate nickname, since it technically is short for Izuku.
All for One is an evil spirit who was able to give blessings and curses to many people at once, which gives him an awesome amount of power since patrons gain more power the more contracts they have. In human history, they've greatly influenced and inspired stories of cryptids and folklore creatures before they revealed themselves to the world en masse.
Eri is a young but powerful familiar that Overhaul captured for malevolent purposes.
Class 1A figures out magical girl names before the Sports Festival. In this universe, they're more focused on secret identities since a patron giving you blessings or curses will usually change your appearance somewhat via magical girl transformation.
