Summary
Instead of eating Hannibal as promised, Mason Verger decides the Alpha would be better suited to being a prized Alpha stud on his farm, perfect for harvesting his seed and selling to other interested parties or for breeding omegas.
When Cordell realizes that Will Graham's heat has begun and that he is not, in fact, a Beta, Cordell and Mason decide Will would be the perfect breedable omega to break the new Alpha stud in. They threaten to kill Will if Hannibal refuses to mount and breed him.

Hannibal Lecter becoming Will’s psychiatrist after the Hobbs shooting was a near miss.
Instead, Will managed to insist Alana became his therapist, and Jack brought Hannibal Lecter on as a consultant on the Chesapeake Ripper instead - and he immediately made his interest in Will known.
The problem with Hannibal Lecter, Will decided, wasn’t so much that he was an arrogant, overdressed, snobbish Alpha psychiatrist, but that he had no right smelling as good as he did. Will had felt a sharp stab of attraction, walking into Jack’s office the first time they met, which was not like him at all.

Will is an Omega who has been on heavy suppressants since he was a teenager, enabling him to pass as a Beta. His attempt to convince himself and everyone else that he is, in fact, a Beta has been successful until he falls ill with encephalitis and his psychiatrist, the purebred Alpha Hannibal Lecter, urges him to stop taking the hormonal suppressants. Barely a week after he stops taking the prescription, he goes into heat for the first time in his life. Hannibal is pleasantly surprised to find the scrappy Omega on his doorstep, wet and aching for an Alpha. (Loosely based on the manga series Love is an Illusion by Fargo.)
