Before Tanjiro could reply, the male spoke again. “Can you tell both of them—Hizashi and Shouta that I’m proud of them.”
He exhaled deeply, as if that was something he had been holding for a while.
Hearing the sincerity and emotions in his voice, Tanjiro responded, “ok.”

After the final battle, Tanjiro is transformed into a demon. And without a cure, dies shortly afterward.
Tanjiro wakes up and takes his first breath in the my hero world
After nine months under the care of two pro heroes, Tanjiro encounters a mysterious stranger who claims to be an old of his adoptive parents.
He gets asked to relay a message to them,
and he does.
Hizashi and Shouta are shocked when their newly adopted son re open old wounds

Summary
Megumi blinks open his eyes. Assaulted with the bright white of a hospital room, he almost wishes he hadn’t, but nonetheless turns his gaze to whoever is with him. A big burly man, hair so red that Megumi assumes it’s dyed, stands before him, young but skin dug into with frown lines and faint stress wrinkles. Megumi half jokes to himself that the man might be worse off than Nanami.
He opens his mouth to speak, the question of what happened and where Gojo is on the tip of his tongue. Only when he attempts to voice his words, nonsensical babbling replaces his speech and Megumi wants to scream.
OR: Fushiguro Megumi dies at the hands of his mentor inorder to prevent the King of Curses - Ryoumen Sukuna - from destroying and enslaving the world.
He is reborn as Todoroki Shouto, son and number 1 hater of Endeavor, wondering why he couldn’t just die in peace.
