Luffy, Zoro, and Sanji are caught in a major twist of fate where one of their senses becomes devastatingly sensitive, twisting them into torment that isolates them in every possible way. Chopper turned to face him, and he was met with a pitiful sight. Luffy was sitting on the ground, holding his left elbow. His eyes were wide, brows furrowed as if in pain, and mouth agape. He seemed to be muttering something under his breath that Chopper couldn’t hear. The doctor approached him slowly, hesitantly, bracing himself for what he will learn next. “Luffy, do you need…” he didn’t finish his question because his voice didn’t call Luffy’s attention. He looked to be entranced, gazing out at sea but nothing in particular as words quietly came floating out of his mouth.“...rry…so sorry…”
“Luffy?”
“I’m so sorry, Nami.”

Emetophobia Warning~!
“I am hurt,” she gritted out, hunched over, cramps spiking, saliva pooling in her mouth. She swallowed it. She wasn’t doing this. It didn’t matter if they knew or not. She needed to get to her room and bleed out in peace. “It’s my time of the month.”
Usopp’s eyes widened, Sanji gasped, and Zoro’s hand flew to his sword hilts. Luffy cocked his head.
“Time of the month?” He thought about it for a second, before breaking into a smile. “Oh, like birthdays! That’s why Nami wanted candy! Sanji, let’s have cake!”
Sanji stuttered something out, and the saliva was back, the last warning Nami got before she was scrambling for the railing, violently vomiting into the sea.
Or, Nami gets her period for the first time since joining the Straw Hats and it goes as wonderfully and as horribly as one might expect.

Sanji straightened his tie, smoothed back his hair, and smiled at his reflection in the mirror. The other Sanji, the one with black hair and cruel eyes that lived in his mind, said, “Do you ever wish you were different in every single way? Because you should.”
But Luffy said, “You’re a good cook.”
And Sanji smiled at the compliment even though Luffy knew he didn’t really understand it.

Summary
Sanji liked to stay on the positive side when it came to other people and their odd little behaviors and quirks, but it didn’t seem quite right to do it for his own. It was easier to think in terms of debt and survival and penance than focusing on the positives.
The mouse sitting on his shoulder, observing his meal prep, squeaked at him, and Sanji smiled.
This indulgence was done in honor of all the poor mice friends who had been killed because he showed them kindness. When Sanji reached his hand up to offer scraps too small to save for Luffy, the mouse nuzzled his fingers.
No other reason.
