Among Dottore's segments, the youngest had always been the odd one out. Barely eight years old, he was small in stature and easily overlooked at first glance. Yet, his mind was no less brilliant than that of the older segments.
Proof was how he managed to have Scaramouche wrapped around his little finger.
Or, Segment 8 was undoubtedly Scaramouche's favorite.

Summary
Il Dottore appears in a world where “heroes” are a social pillar and quirks are something people are born with.
He, in particular, never believed in heroes or villains, only in poorly designed systems and in how easily human ambition drives them to collapse.
Forced to exist in a world that idolizes heroism, he doesn’t try to adapt or seek redemption. He studies its rules, finds its flaws, and begins to intervene as someone who doesn’t see people, but pieces. His actions don’t go unnoticed, and when the right villains set their sights on him, the balance starts to shift.
The line between villains and saviors becomes irrelevant, and the real problem stops being who is fighting… and becomes who is controlling the board.

fate has decided death is somehow still too cruel, and so he's saved by someone who has a complete different world view than him and is dead set on making sure he's hydrated and fed.

This was it for him. He could feel his consciousness slipping away. He imagined it pooling like blood outside his body on the floor in front of the Tsaritsa’s throne. Ajax closed his eyes. Voices continued to float around him, but he was too far gone to care.
“...Your Majesty. If I may… since you no longer have use for him… he would make an exceptional test subject…”
He hoped someone would tell his family what happened to him. He couldn’t bear to imagine Tonia and Teucer– poor, sweet, naive baby Teucer– waiting for his return.
“Thank you, Your Majesty,” he heard as he took a final, shuddering breath.
Everything went dark.
Rebirth at the hands of The Doctor is a fate worse than death.
