Apparently, Cas can now only heal with kisses. Sam smells something fishy, of course, but there’s no way he’s getting involved; the unresolved tension between Cas and Dean could probably fuel at least ten monster trucks—provided monster trucks ran on weird, sexually-frustrated, interspecies relationships—and it’s about time one of them finally did something about it.
